thanks so much for doing this. Comments as I filled in the form. Hope they help. - Name field could be bigger
- Second address line and the following address lines confused me a bit. Can you control format?
- Birthdate day/year field lets me put in not standard info, as I could not get the calender to appear. DB entries won't appear uniform in final list
- Second Interests, match wording, state as second interest

Well the form is definitely working, feel free to fill it out...eventually very member will have to. I would also like to have paper forms available at the meetings, if possible.
I tried to fix some of your concerns Tom, but it is a free service and some things can not be changed. I'll work on it some more.
I have started an Excel file and am inputting the information manually right now. This job will go to the Membership Secretary when we get one and they will be able to change and do things however they want.

The membership form is up and running. Please take some time and fill one out and submit it. This is going to be very important for when we get up and running. I am posting on FB and sending out email reminders to those I have email addresses for.
